They do team debriefs after codes. Psychological safety to discuss what went wrong.

Worked here during Joint Commission survey week. The anxiety was palpable. But honestly, the hospital was already doing things right so it wasn't the scramble I've seen at other places. The fact that they passed with minimal findings tells you something about their baseline standards. Props to the quality team here.
